Online Annual Asbestos Quarterly Report Instructions/Help
Do you have multiple on-going asbestos removal projects during a calendar year. Use this web form apply for an Existing Annual Notice. Simply:
- A new Notice must be requested each calendar year in which activity occurs
- The case number will be retrieved if the case is found otherwise a case not found message appears
- Use the guides below to assist in completing the form
- A fee of $684.00 is required for submitting an Annual Notice
- Review your fields for accuracy
- Click the reCaptcha box
- Complete the Challenge Pictures if requested
- Click the SUBMIT button
Quarter/Coordinator
Select the Quarter and Year for this report. The Year and Quarter selected must match the Annual Notice selections. Enter the name of
the person responsible to coordinate the asbestos removal. Enter that person's phone number in case we have to contact him/her.
First, Second, Third Jobs
Up to 3 jobs may be reported using this form. Re-request the form for additional sets of 1-3.
- Enter the your job number or description. This is a free-form data field that identifies your process to occur
- Enter the area containing asbestos (e.g. hallway, boiler room, basement, etc.)
- Enter a short name of the equipment or object containing asbestos
- Select the Asbestos Contractor from the list. Enter the company License Number
- Enter the name of the on-site contact person
- Select the type of material containing asbestos
- Enter the quantity to be removed of either Square Feet or Linear Feet
- Enter the estimated project start and completion dates
- Select the disposal site and date of material transfer
Repeat the above steps for each job to be submitted
Verify and Send
To prevent erroneous submissions from automated spam processes, click the check box in the reCAPTCHA box. If presented with a challenge picture,
select the panels that correspond to the question asked. Upon successful completion the check box will be replaced with a green check mark.
After you review your content, click the SUBMIT button at the bottom of the form. Your entries will be validated and if there are errors a red error
box appears at the top of the page. Correct the fields listed in the error box and click SUBMIT button again.
After a few moments you will be directed to a confirmation page. If there are no process errors an email
notice of your request is sent to our staff. If an error occurs during processing an error message appears instructing you as to what
needs to be done to complete your request.
